Insurance Plans
XRC Medical Imaging and X-Ray Consultants, Inc., are approved providers for most major insurance carriers and health plans, including Medicare and Medicaid. We are in-network for all contracts held by Select Health Network. We will submit both primary and secondary insurance claims to insurance companies on our patients’ behalf.
Coverage may vary depending on the service location (imaging center vs. hospital). Any questions should be directed to your insurance carrier or to our billing office, at (574) 233-3123.
To streamline the registration process and to minimize your personal expenses, it is important to bring the following information with you at the time of your appointment:
- Your insurance carrier’s policy regarding pre-authorizations or referrals.
- Your complete insurance information, including:
- Insurance card
- Name and address of the plan or insurer(s)
- Name and address of the claims payer (if different from insurer)
- ID and group numbers
- Name of the policyholder or subscriber
Prior to your exam, an XRC Medical Imaging representative may call to assist you with pre-registration. Please have the above information available and refer to the list below to verify our participation with your insurance.
