Your Medical Records

All of your medical records, your images, your reports and any medical, demographic, insurance or personal information that you provide to us is held in the strictest of confidence and in keeping with the highest standards of medical ethics and applicable law.

XRCMI is a filmless institution for all modalities except mammography. That means that we do not record your images on traditional x-ray film. Instead we view and store your images electronically, along with the reports of those images. We make your images and reports available to your physician in a variety of ways, via a secure web site, by FAX, on CD-ROM, on paper or even on traditional film, if he or she so requests. Keep in mind however, even if we do provide your physician with film images, we do not retain those images on film at XRCMI.

Should you need a copy of your images or reports to go to another physician(s), we will provide them directly to those physicians free of charge with your written consent. If you want copies for your own personal use, please contact your physician first; he or she may be willing to allow you to have their copy (if they had requested a hard copy from us). If that is not possible, XRCMI can print you copies at a moderate charge. With regard to legal matters, your attorney may obtain copies of your images and records from us with your written authorization.

XRCMI does provide obstetrical ultrasound patients with "baby pictures", images of their children to be, at no charge. From time-to-time we may also provide free images of certain other types of exams on a promotional basis. Please understand that the costs associated with providing our patients with their own set of images on a wide-scale basis however, is simply prohibitive and would prevent us from our goal of providing the highest quality services at reasonable cost.

If you have any questions about out medical records policies, please call us at (574) 243-0100 or click here to contact us.
