Medical Imaging cannot accept self-referrals for any study other
than screening mammography. Therefore, any other type of exams
must ordered by your doctor. As a matter of regulatory compliance,
we will be asking your doctor for a signed, written order for
your exam(s) along with a diagnosis code and any relevant signs,
symptoms and allergies that you might have. We will also attempt
to gain necessary demographic and insurance information at that
time.
We may also attempt to contact you prior
to your appointment to confirm this information and / or to
obtain more information for your preregistration. Depending
upon the type of procedure you are scheduled for, there may
be preparation instructions that we can review with you at
this time.
Please try to arrive 20 minutes prior to
your appointment to verify all of the information and to sign
required forms. Your early arrival will help us to stay on
schedule, which is very important to us as well as yourself
and other patients.
