Insurance Plans
X-Ray Consultants, Inc., is an approved provider for most major insurance carriers and health plans, including Medicare and Medicaid. We are in-network for all contracts held by Select Health Network. We have a full-time Insurance Coordinator that will submit both primary and secondary insurance claims to insurance companies on our patients’ behalf.
Coverage may vary depending on the service location (imaging center vs. hospital). Any questions should be directed to your insurance carrier or to our billing office, at (574) 233-3123.
To streamline the patient registration process and to minimize patient personal expenses, it is important that the patient bring the following information to their appointment:
- Patient’s insurance carrier’s policy regarding pre-authorizations or referrals
- Patient’s complete insurance information, including:
- Insurance card
- Name and address of the plan or insurer(s)
- Name and address of the claims payer (if different from insurer)
- ID and group numbers
- Name of the policyholder or subscriber
Prior to the patient’s exam, an X-Ray Consultants, Inc. representative may call to assist them with pre-registration. Patients need the above information available and can refer to the list below to verify our participation with their insurance.
